Eligibility and Forms
Eligibility
A student must be eligible academically to participate. All eligible players will have passing averages for four out of their five classes from the previous semester (in this case, from January of the previous school year to May of the previous school year). The student has four academic classes and the fifth class will be the average of their connection classes.
Forms
To practice, you must have the following forms:
1. Current Physical (must have doctor's signature)(Spanish)
2. Insurance Waiver (complete online)
3. Concussion Acknowledgement Form (complete online)
4. Sudden Cardiac Arrest Form (complete online)
